Attached is a list of the questions and information needed to complete Forms Two and Three. Please use this sheet to help gather necessary Form Two and Three information before you sit down to file the forms. Forms Two and Three will become available at the time of the test. Only those filers who have previously submitted Form One for the current test cycle will be able to submit Forms Two and Three in ETRS.
